-If the Parent/Legal Guardian's or Applicant's name does not match the name on the Parental Relationship Documents you must provide evidence of a legal name change.
-The Parental Relationship Documents will be mailed off but will be returned to you at the same time as the U.S Passport.

If you have your own current identification document (ID), you may apply for a passport. You must show at least one parent or guardian knows that you are applying for a passport. Your passport application may be denied if we have received written objection from one of your parents or legal guardians.
How to Show Parental Awareness::
You must submit one of the following U.S. citizenship documents for your minor. Your evidence must be an original or certified copy. Photocopies and notarized copies are not acceptable.
